Lung cancer isn't just a smoker's disease: Doctor explains how air pollution, genetics, and everyday exposures may quietly increase your risk

For years, lung cancer has carried one overwhelming assumption: it happens only to people who smoke. That belief has become so deeply rooted that many non-smokers ignore persistent symptoms, believing they are "not at risk." Doctors say that mindset needs to change.Across the world, specialists are seeing more cases of lung cancer in people who have never touched a cigarette. While smoking continues to be the biggest risk factor, it is no longer the only one that deserves attention. The air people breathe, the places where they work, their family history, and even invisible changes in their genes can all influence the likelihood of developing the disease.As environmental pollution continues to rise in many cities and scientific research uncovers more about how cancers develop, understanding lung cancer has become far more complex than simply asking whether someone smokes.Air pollution does much more than irritate the eyes or trigger a coughing spell on a bad pollution day. Tiny particles suspended in polluted air, especially PM2.5, are small enough to bypass the body's natural defence mechanisms and travel deep into the lungs. From there, they may even enter the bloodstream.Once inside the body, these microscopic particles can trigger chronic inflammation and oxidative stress. Over many years, repeated damage may affect the DNA inside lung cells. Although the body constantly repairs damaged cells, long-term exposure can overwhelm these repair systems, allowing abnormal cells to grow.Vehicle exhaust, industrial emissions, construction dust, burning crop residue, diesel fumes, and smoke from biomass fuels used for cooking all contribute to this exposure. Indoor air pollution also deserves attention, particularly in homes that rely on wood, coal, or dung cakes for cooking in poorly ventilated spaces.The World Health Organization (WHO) classifies outdoor air pollution and particulate matter as carcinogenic to humans, meaning there is sufficient evidence that long-term exposure increases the risk of lung cancer.The risk does not appear overnight. It builds slowly, often over decades, making clean air a long-term health investment rather than simply an environmental issue.Dr Surender Kumar Dabas, Chairman, Manipal Comprehensive Cancer Centre and Onco Robotic Surgeries, Northwest Cluster, Manipal Hospital, stressed that smoking continues to remain the leading cause of lung cancer, but recent trends are changing how doctors view the disease."For decades, smoking has been recognised as the leading cause of lung cancer. While tobacco use continues to account for the majority of cases, today we are witnessing a concerning rise in lung cancer among people who have never smoked. This changing trend highlights that lung cancer is no longer solely a smoker's disease, making it important to understand the multiple factors that contribute to its development."He further explained that tobacco smoke directly damages lung tissue over time."Smoking remains the single biggest risk factor, with the chemicals in tobacco smoke damaging lung cells over time and increasing the likelihood of cancer. However, second-hand smoke also poses a significant threat, particularly for individuals exposed to it regularly at home or in the workplace."Second-hand smoke contains many of the same cancer-causing chemicals inhaled by smokers themselves. Children, spouses, and co-workers who breathe this smoke repeatedly may also face higher health risks despite never smoking.This is one reason why smoke-free homes and workplaces matter. Protecting non-smokers is not just about comfort, it is about preventing disease years down the line.One of the biggest advances in lung cancer care has been the understanding that genetics can influence who develops the disease.Some people inherit genetic variations that increase their susceptibility, while others develop acquired gene mutations during their lifetime. These mutations can encourage cancer cells to grow even in people with no smoking history.Dr Dabas explained, "Genetics also plays an important role. Some individuals inherit genetic traits that make them more susceptible to lung cancer, while others develop specific gene mutations that drive the disease despite having no history of smoking. Identifying these mutations through molecular testing has transformed treatment, allowing patients to benefit from targeted therapies that are more precise and effective."Doctors today routinely test many lung cancers for specific gene changes because the results can help determine the most effective treatment. Targeted therapies have improved outcomes for many patients whose cancers carry these mutations.Other environmental risks should also not be overlooked. Exposure to asbestos, silica dust, radon gas, and certain industrial chemicals can increase lung cancer risk. Previous lung diseases, including pulmonary fibrosis and tuberculosis, may also contribute in some individuals.Lung cancer may not cause obvious symptoms in its earliest stages. That is why many people receive a diagnosis only after the disease has advanced.According to Dr Dabas, "Persistent symptoms such as a cough lasting more than three weeks, breathlessness, chest pain, coughing up blood, or unexplained weight loss should never be ignored. Early diagnosis significantly improves treatment outcomes, thanks to advances in imaging, minimally invasive surgery, robotic surgery, immunotherapy, and targeted treatments."He also highlighted the importance of screening for those at higher risk."Screening plays an important role in detecting lung cancer at an early stage. Low-dose computed tomography (LDCT) is currently recommended for individuals at high risk, particularly long-term smokers and former smokers with a significant smoking history. It can detect small lung nodules before symptoms appear, improving the chances of successful treatment.
